Abstract

Introduction: Tuberculosis represents a public health problem in Brazil and in the state of Pernambuco, as it is a non-random disease that predominantly affects socially vulnerable populations. Objective: This study aimed to describe the distribution of tuberculosis among the Health Macroregions of Pernambuco between 2020 and 2023, analyzing incidence according to sociodemographic variables and clinical outcomes. Methods: This is an ecological study based on secondary data from the Notifiable Diseases Information System (SINAN). Results: The Metropolitan Macroregion accounted for approximately 81% of tuberculosis cases in the state. The results showed higher rates among men, adults aged 20 to 59, and the elderly, especially among the brown population. There was an increase in incidence in all macroregions during the study period. The Metropolitan Macroregion had the lowest cure rate and, consequently, the highest dropout rates, highlighting gaps in continuity of care. Conclusion: The data indicate a higher incidence in more urbanized and impoverished regions, as well as an increase in cases among the elderly, young adults, and people who, due to living in contexts of poverty, racism, and territorial exclusion, have historically had their right to health denied.
